Yep that about says it all. At times in my work place the paper work and permit system is so complex and cumbersome that it's creating incentives for people to short circuit the system and do tasks without the proper risk analyses and risk mitigation......

Getting to the stage here at work where you just about need to draw up a risk analysis to itch yer bum! Still maybe not a bad thing seeing as we are Chemistry & one unlabelled jar could potentially blow a few walls out!
